Account deletion request

Once you submit this request, the following process begins.

Key points

90-day deactivation period: Your account is deactivated immediately and your personal account data is permanently deleted after 90 days.
What is deleted: Your login, profile, contact details, preferences and app activity linked to your Moana account (MOANA Mobile, Patient Portal or MOANA Desktop).
Clinical records: Health records created by your healthcare provider are held on behalf of that provider and are kept for as long as the law requires. To request access to or correction of these records, contact your healthcare provider.
Access revocation: You will lose access to the Moana apps and portal for this account.
Irreversible action: Once the 90-day period ends, deletion cannot be undone and account data cannot be recovered.
